Assistance Program (EAP)Position SummaryFoster Care Case Planner
needed for our NEWLY expanded program! Provide leadership in
partnering with families and appropriate others in the community to
assure that the needs of a designated caseload of children and
youth in foster care are adequately addressed. In each case the
Foster Care Case Planner will assure that the services delivered
both to the target child/adolescent and to the family are
individualized, appropriate to need, child centered, family
focused, culturally competent, strengths based, and goal
driven.Essential ResponsibilitiesAssist and support in training
activities for resource parents.Provide 24-hour emergency "on-call"
service to resource families according to designated scheduleAssist
in the arrangement of respite services.Maintain all necessary
documentation to ensure compliance with licensure and
regulations.Develop and maintain an individual treatment plan for
each child that utilizes formal and informal supports, addresses
clinical and educational needs, and includes measurable and
objective goals.Provide overall leadership and coordination of the
treatment team.Coordinate all community resources pertaining to
discharge placement and arrange aftercare follow up if
indicated.Assure that all policies, procedures, and standards
relating to child safety and well-being are followed.Assess and
identify the needs of the birth family to achieve their family
service plan goals.Actively participate in individual/group
supervision, staff meetings, and professional trainings to foster
ongoing professional growth.QualificationsClearances. State Police;
FBI clearance; child abuse clearance; CPSL Mandated
Reporter-Recognizing and Reporting Child Abuse training; any
additional background checks/clearances required by state governing
bodies.Valid driver's license and current vehicle insurance.Working
ConditionsPhysical Demands. Requires vision, speech, and hearing
(exceptions are made for deaf or hearing impaired staff that can
communicate effectively with hearing staff and
families)Environmental Factors. Community, home, school,
office.Working Hours. Flexible non-traditional hours, evenings,
weekends, and some holidays. A non-traditional work schedule as
defined by service needs and crisis "on-call". Full-time positions
are based on a 40-hour work week.ExperienceRequired1 year(s):
Supervised experience with children and/or
