Acute Care Hospital Trauma and Stroke Accredited; 60 Critical care
beds including specialty neuro-trauma intensive care; consultations
completed for both symptom management and advanced care planningJob
DescriptionPOSITION SUMMARY: The qualified candidate will possess
broad clinical knowledge and excellent interpersonal and
communication skills. Clinical responsibilities include expert
management of physical and emotional symptoms related to serious
illness or disease-directed treatment, facilitation of advance care
planning discussions, transition planning, and close collaboration
with each patient's primary team. There is tremendous support from
hospital leadership to expand access to high-quality primary and
specialty-level palliative care across care settings through
scalable models of care delivery. The candidate will directly
report to the Medical Director of Palliative Care, and will have
the opportunity to participate in palliative care education and
quality improvement initiatives throughout the health system.
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability
(with or without reasonable accommodation) to perform the following
duties: Consults with patients to determine the appropriate course
of treatment. Recommends and orders tests to determine extent of
illness or help diagnose condition. Monitors lab results,
medication adherence and medication toxicities. Prescribe or
administer treatment, therapy, medication, and other medical care
to treat or prevent illness, disease, or injury. Explain and
discuss procedures, test results, or prescribed treatment with the
patient. Coordinate work with nurses, social workers, Chaplains,
pharmacists, psychologists, and other health care
providers.SECONDARY FUNCTIONS: The following duties are considered
secondary to the primary duties listed above: Represents the
practice at LGH committees in such a manner that the burden of
attending the meetings is reasonably distributed among all of the
physicians in the group. Assists in recruiting and interviewing of
new physicians and staff to the practice. Attends Committee
meetings. Acquires the necessary CME credits and takes the
certification examination to maintain re-certification status.
Participates in the on-call responsibilities for this position as
required. Other duties as assigned.JOB REQUIREMENTSMINIMUM REQUIRED
QUALIFICATIONS:
- Medical Doctor (MD), Doctor of Osteopathy (DO), issued by the
Accreditation Council on Medical Education (Allopathic /
Osteopathic).
- Completion of an ACGME accredited Hospice and Palliative
Medicine Fellowship
- Board Eligible or Board Certified within two years in Hospice
and Palliative Care
- Current Medical licensure issued by PA State Board of Medicine
or the PA State Board of Osteopathic Medicine.
- A health care provider in good standing with Medicare,
Medicaid, and other federal and state health insurance programs,
i.e. not excluded from participation in Medicare, Medicaid or any
other fede ral or state health insurance program.PREFERRED
QUALIFICATIONS: -
- 2-3 years of experience as a Hospice and Palliative physician
