Verizon Wireless Discount Position Description: The Substance Use Counselor provides direct counseling and case management services to patients, consisting of individual, and group services, including assessment, individualized treatment plans, individual and group counseling, and discharge planning. Duties and Responsibilities: Maintain caseload per State Regulations
Assures incoming patients have psychosocial assessment and an interpretive summary according to the timeline in the ARS Policies and Procedure Manual
Completes, with patient participation, a Preliminary Treatment Plan according to the timeline in the ARS Policies and Procedure Manual
Completes a Treatment Plan Review and update for each patient after completion of the Comprehensive Treatment Plan according to the timeline in the ARS Policies and Procedure Manual
Prepares a discharge transition plan for all patients according to the timeline in the ARS Policies and Procedure Manual
Provide coordination of care for patients as needed
Adherence of 85% patient engagement
Satisfaction rating of 85% or better in patient satisfaction surveys
Be proficient with State Licensure Standards, CARF Standards, 42 CFR and HIPAA standards and Company policies
Adherence to Mission and Vision of the company Requirements and Qualifications Associate degree in related field with 2 years clinical experience
Bachelor's degree in related field with 1 year clinical experience
Master's degree in related field with practicum
CADC/CAADC, certified or written plan to become certified Pay Frequency: Bi-Weekly
