Job Description Job Description Individualized Planning &
Service Oversight - Develop, review, and approve Individualized
Service Plans (ISPs) in collaboration with individuals served,
families, case managers, and interdisciplinary team members. -
Ensure ISPs are person-centered, outcome-driven, and aligned with
assessed needs, strengths, and preferences. - Monitor ISP
implementation to confirm services are delivered as written. -
Conduct required ISP reviews, updates, and team meetings within
mandated timelines. - Caseload will consist of maximum of 30
individuals. Quality Assurance & Compliance - Lead agency-wide
quality assurance activities related to DDA and Medicaid-funded
services. - Conduct routine and unannounced chart audits to ensure
documentation accuracy, timeliness, and compliance. - Monitor
compliance with regulations related to individual rights, health
and safety, incident management, and service delivery. - Identify
trends, risks, and systemic issues; develop and track corrective
action plans. - Ensure readiness for licensing inspections, audits,
and quality reviews. Documentation & Data Integrity - Review daily
notes, ISP data, incident reports, and service documentation for
compliance and quality. - Ensure documentation supports billing,
service authorization, and regulatory requirements. - Maintain
organized, audit-ready records in electronic and paper systems. -
Track compliance deadlines and reporting requirements. Staff
Training & Performance Support - Train DSPs and supervisory staff
on ISP implementation, documentation standards, and compliance
expectations. - Provide corrective feedback, coaching, and
technical assistance related to quality and regulatory adherence. -
Support onboarding and annual retraining initiatives. - Ensure DSPs
do their work in accordance to standard operation. - Provide
coverage when there is shortage. Incident Management & Risk
Oversight - Review, investigate, and follow up on incidents in
accordance with DDA and agency policy. - Ensure timely reporting,
documentation, and implementation of preventative measures. -
Monitor health, behavioral, and safety risks and escalate concerns
appropriately. Communication & External Coordination - Serve as
primary liaison with case managers, families, consultants, and
regulatory entities regarding compliance and quality issues. -
Participate in interdisciplinary team meetings, quality improvement
meetings, and leadership reviews. - Provide regular compliance and
quality reports to agency leadership. Required Qualifications -
Bachelor’s degree in human services, Social Work, Psychology,
Sociology, Special Education or related field as required. - Meets
DC and Maryland DDA qualifications for a Qualified Intellectual
Disabilities Professional. - Minimum of one (1) year of experience
working with individuals with intellectual and/or developmental
disabilities. - Demonstrated knowledge of DC/Maryland DDA
regulations, Medicaid HCBS requirements, and ISP standards. -
Strong analytical, organizational, and documentation skills. -
Ability to interpret regulations and apply them to daily
operations. - Proficiency with electronic health record and
documentation systems.
