Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 2 years required License: Medical license required Certifications: BLS required Must-Have: Discharge planning experience required. Ability to handle up to a 20 patient assignment per day. Acute hospital experience required. Travel experience required. Every other weekend required. Float to medical units, ICU, CCU, and maternal child required. Description: Seeking a Case Manager RN. Open to LMSW, MSW, or LCSW. Locals within 60 miles accepted at a lower rate. Discharge planning experience is essential for this role. Candidates must manage a patient load of up to 20 patients daily. A current medical license is mandated for this position. BLS certification is required. Acute hospital experience is essential, with a minimum of two years required. Travel experience is necessary. Weekend coverage includes two 2-day weekends in a 6-week rotation. Position requires floating to medical units, ICU, CCU, and maternal child. Inpatient case management experience is needed.
