- Education: Graduate of an accredited school of nursing; BSN
preferred
- Licensure: Current and valid Registered Nurse (RN) license in
the state of Pennsylvania
- Certification: Certified Wound Ostomy Continence Nurse (CWOCN)
certification required
- Experience: Minimum of two years of experience as a Registered
Nurse in a relevant clinical setting; prior experience in wound
care, ostomy care, and/or continence management preferred
- Skills: Excellent assessment, critical thinking, and
problem-solving skills; strong interpersonal and communication
skills; proficiency in electronic medical records systems; ability
to work independently and as part of a team; current CPR/BLS
certification; valid driver's license and reliable
transportationPosition Details
- Responsibilities:
- Conduct comprehensive assessments of patients' wounds,
ostomies, and continence issues
- Develop individualized plans of care in collaboration with
physicians and interdisciplinary teams
- Select appropriate wound dressings, therapies, and equipment
based on evidence-based practice guidelines
- Educate patients and families on wound care techniques, ostomy
care, and continence management strategies
- Provide direct patient care, including wound dressing changes,
ostomy care, catheter care, and skin care
- Monitor patient progress and adjust care plans as needed
- Provide emotional and psychosocial support to patients and
families
- Participate in quality improvement activities related to wound
care, ostomy care, and continence management
- Maintain accurate and timely documentation of all patient
