Job DescriptionResponsible for oversight of home, hospital, and nursing home care. Completes audits at the nursing home, home care agency, and participant's home per the Contract Oversight and Quality of Contracted Services policy and procedure. In addition, coordinates all home care services including all nursing home care under the supervision of the Program Director and Registered Nurse Home Care Coordinator.Provide skilled nursing care and prescribed treatments to participants in the participants home demonstrating quality care delivery by physicians' orders; directing and supervising the provision of quality para-professional clinical care to participants in their homes; participating as an on-call nurse by the Albright LIFE on-call policies and procedures.
Qualifications

  • Must be a Licensed Practical Nurse currently licensed in the Commonwealth of Pennsylvania with one year experience as a licensed practical nurse with a frail or elderly population.
  • Current Pennsylvania Driver's License
  • Thorough knowledge of current community health nursing practice and skill in the application of current nursing procedures
  • Ability to effectively implement plans of careand coordinate nursing care for individuals, families, and groups.
  • Ability to work independently and as part of a team, under pressure, while maintaining cooperative working relationships
