Shifts: Days 7a- 7p , Nights 7p-7a
RN provides direct nursing care to patients as prescribed by the
physician & is under the direction & supervision of the appropriate
facility nursing leadership. The RN provides nursing care specific
to the age population served. Nursing activities are limited to
those in compliance with the State RN Practice Act & facility
policies & procedures policies/procedures
Conducts ongoing & individualized nursing care duties as assigned &
as dictated by the pt's condition in accordance with physician
orders & facility protocols.
When administering PRN medications, assess for confirmed med for
PRN, documents need (rationale) & its effectiveness in the medical
Reports change in pt physical, mental status or episodes of
seclusion or restraint to RN manager or supervisor during each
shift. Ensures family & physician notification is completed &
documented if a pt is transported to Emergency Room, Injured, Falls
or is Placed in Seclusion or Restraints.
Collaborates with other team members in the development,
implementation, & evaluation of an individualized plan of care
Performs treatments & therapies as ordered by physician in an
appropriate & timely fashion.
Documents pt care, findings, & psychosocial responses to nursing
Documents PRN medications given, reason & effect
Initiates emergency measures according to age appropriate CPR &
client resuscitation protocols.
Participates in emergency crisis interventions according to age
Provides pt care in a non-judgmental, non-discriminatory manner
that considers cultural diversity & age appropriateness so that
autonomy, rights, & dignity are preserved.
Maintains competency by participating in continuing education
programs & meets state specific requirements.
Complies with accepted ethical conduct & professional Standards of
Nursing Practice as set forth by the American Nurses Association or
equivalent national organization.
Consistently engages and interacts with our patients and their
families by using the treatment principles of Trauma Responsive
Care, as demonstrated by the following:
A basic recognition of the characteristic symptoms resulting from
exposure to trauma,
Having an understanding of and sensitivity to the triggers of
trauma and trauma re-enactment
Incorporating relevant skill sets into daily interactions with
patients and families, and
Participates in continuing education on Trauma Responsive Care and
Complete nursing admission assessments and daily nursing
Transcription of medication orders in a timely, error free manner,
and ensuring reason for PRN medication is documented.
Complete Adverse Drug Reaction forms and forward to
Complete First Dose form as ordered by physician.
Ensure order clarification of non-formulary medication if ordered.
Obtaining appropriate order.
Provide patient education related to medications and document
Assist patients in carrying out activities of daily living.
Participate in group activity with patients.
Ensures room searches are done for contraband during shifts.
Direct and assist mental health clinicians in the care and
direction of patients.
Assists in the completion of patient rounds and monitoring patient
Participate in hospital emergency codes necessary to the unit's and
the hospital's safe operation.
Will adhere to all safety policies and safe work practices.
Complete 24 hour chart checks, ensure filing is completed, charts
are stamped, MARs are copied correctly on 11-7 and shift chart
checks on 7-3 and 3-11.
Graduate of an approved School of Nursing. ASN or Diploma Nursing
Licensure: Current and unrestricted license as an RN in the state
Experience: At least one year experience as a RN in a clinical
setting is preferred.
Credentials: Current CPR
Additional Training: Handle With Care training is required
